Must Wear A Face Mask
Anyone entering the clinic must wear a face mask properly covering the nose and mouth at all times.
Patient Check In Survey
All appointments must fill out a “patient check in survey” which requires a temperature check
Social Distancing
Social distancing-6 feet apart is recommended.
- Walk-in clinic suspended till further notice.
- Hand sanitizer spray available for every patient in clinic.
- Every patient entering the clinic for an appointment needs to fill out a” patient check in survey” form and wear a mask (if accompanied by a spouse or guardian, they too must fill out a patient check in survey form).
- Patients with symptoms such as cough, cold, congestion, a survey needs to be filled out and addressed to the provider they are seeing for approval of entering clinic appointment may be changed to a telemed appointment.
- Patients with covid or exposed to covid must call clinic before entering.
- For the safety of all-if any answers change while in exam room, the provider may direct patient to a telemed appointment.
